I would never have got this, and unless you are a "Psycho aficionado", I doubt anyone would guess this. It is such a trivial thing in the movie.

  It was the first American motion picture to feature a toilet being flushed.

Aaaa....yea.  That would be the answer.  Someone would surely get it once the clues are given.  That's part of the game.  You don't necessarily have to be a "Psycho' aficionado to get this.....a trivia buff might know this answer without even having seen the movie.
